The introduction of high risk HPV testing as the primary cervical screening tool may possibly lead to earlier identification of more glandular pre-invasive lesions than presently is the case with cytology based screening.

The identification and management of pre-invasive and early invasive glandular lesions continues to be a challenge for women, and gynaecologists, with many uncertainties as to ideal management.  The proposed guidelines are suggesting some new approaches to management and follow-up of women with glandular cervical disease.  The supporting evidence and safety of these new approaches will be discussed.
